As the hunger grew inside of him, Sam knew that he was going to fall. It was worse than he ever felt in these last months, and he knew as Dean and Cas prepared to hunt Famine, he needed to stay behind.
But when the demons showed up at the door, he knew that there was a choice to make. He had to take them out before they got him. He was in no condition to fight like that... so he made his choice. He fell, and fell hard as he drained not just one, but two demons completely before taking off after his brother with their blood still staining his mouth.
He wasn't going to play their game though. As he stood in the diner and stared down Famine, he saw that the Horseman was trying to tempt him. Sam was supposed to be a good little vessel with that demon blood pumping in him, following Famine behind his little electric Scooter and bow before Lucifer.
"No."
Apparently, they seemed to forget that Sam was a part of Team Free Will. As he looked at his brother's face, seeing Castiel watching him carefully, Sam remembered exactly who he was and what he was here to do.
Save people. Hunt things. Take care of his family.
When it was over, Sam managed to keep himself standing as the hunger started to fade away. His head fell back in blessed relief from that, and then he looked at Dean. He saw the fear in his brother's eyes and just nodded in a silent agreement.
"We need to go to Bobby's. Now."

It hit bad halfway there and Sam knew he couldn't hide it from either Dean or Castiel. At the next fuel stop, Cas and Sam switched spots and Sam just curled up in the backseat, curled up in pain as he prayed for the Impala to just move faster.
"Hold on, Sammy, we're almost there," Dean muttered softly every time Sam couldn't hold back a cry, and finally Castiel took pity on him and knocked him out until they were only a few miles away.
There was no tricking him this time. Sam walked right into that panic room, leaning onto Dean's shoulder as they found the bed Bobby had set up once again for Sam. He tried to focus on his brother, and he felt Dean squeeze his shoulder as he was getting ready to leave.
"Dean..." Sam stopped him, pursing his lips before looking at the restraints. "Better to do it now than later."
"Sam..."
"Either you do it now, or I'm going to have to make you."
The pained look that came across Dean's face didn't escape Sam's attention, but he had been here before and knew what he had doomed himself for the moment he decided to take the first sip. He just begged his brother with his eyes to do it and held his wrist down at the headboard for Dean to lash up.
"I'll be right outside," Dean told him as he finished the last lash. Sam nodded, closing his eyes and just trying not to succumb to the pain. Try not feel the spike of fear that hit him as he heard the door close behind Dean and latch...
... "Hello baby. I know you would remember just how good it felt and come back to me..."
And ignore the demon in the room who straddled his hips and leaned down over him, her lips against his neck before he felt pain in his chest as what felt like a knife stabbed him in the chest and he screamed in pain.
"I missed you too."
